Case-control onderzoek leert dat er een significant verhoogd risico (OR 1,7; 95% BI 1,1 – 2,5) is van hospitalisatie t.g.v. niet-gastro-intestinale bloeding bij personen die tegelijkertijd perorale anticoagulantia en SSRI’s gebruiken, t.o.v. personen die alleen anticoagulantia gebruiken. Une étude cas-témoins nous apprend que l’utilisation concomitante d’anticoagulants oraux et d’ISRS comporte, par rapport aux anticoagulants seuls, un risque significativement accru (OR à 1,7; IC à 95% de 1,1 à 2,5) d’hospitalisation pour cause d’hémorragie dont l’origine n’est pas gastro-intestinale; par exemple les hémorragies intracrâniennes, l’hématurie, l’hémoptysie, l’épistaxie.

Op deze manier zijn indicatoren voor ‘ondervoorschrijven’ (underprescribing) gedefinieerd. Voorbeeld: indien een oudere persoon voorkamerfibrillatie vertoont die langer duurt dan 48 uur en tot een hoogrisico groep behoort, dan is het voorschrijven van perorale anticoagulantia aangewezen, tenzij er een contra-indicatie voor bestaat (in welk geval anti-aggregantia worden voorgeschreven).

C’est ainsi que sont définis les critères de la “sous-prescription” (underprescribing) par ex.: si une personne âgée présente une fibrillation auriculaire d’une durée supérieure à 48 heures et qu’elle fait partie d’un groupe de patients à risque élevé, alors la prescription d’anticoagulants oraux est indiquée, excepté s’il y a une contre-indication (dans ce cas, des antiagrégants seront prescrits).
