Nierinsufficiëntie: de nitrofuranen zijn in dat geval niet doeltreffend (te lage urinaire concentratie), en gevaarlijk gezien hun systemische toxiciteit (vooral perifere neuropathie).
Insuffisance rénale: les nitrofurannes sont dans ce cas inefficaces (concentration urinaire trop faible) et dangereux vu leur toxicité systémique (surtout neuropathie périphérique).

Bij niet-gecompliceerde urineweginfecties zijn nitrofuranen en trimethoprim nog steeds de eerste keuze.
