Patiënten met een gastro-intestinaal ulcus, gastro-intestinale hemorragie, met gastro-duodenaal ulcus en ernstig hartfalen.
Patients présentant un ulcère gastro-intestinal, une hémorragie gastro-intestinale, un ulcère gastroduodénal et une insuffisance cardiaque sévère.

Andere factoren zoals antecedenten van gastro-intestinaal ulcus, het gebruik van NSAID’s of een infectie met H. pylori beïnvloeden eveneens het risico van gastro-intestinale bloeding.
D’autres facteurs tels que des antécédents d’ulcères gastro-intestinaux, l’utilisation d’AINS ou une infection par H. pylori influencent aussi le risque d’hémorragie gastro-intestinale.

Vu l’existence d’un risque d'aggravation de la pathologie sous-jacente, la prudence est de rigueur lorsqu'on administre l'alendronate aux patientes ayant une affection gastro-intestinale supérieure active (telle qu’une dysphagie, une affection de l’œsophage, une gastrite, une duodénite ou un ulcère) ou présentant des antécédents récents (dans l’année précédente) d’affection gastro-intestinale (telle qu’un ulcère peptique), d’hémorragie gastro-intestinale active ou de chirurgie digestive supérieure, autre qu’une pyloroplastie (voir rubrique 4.3).

Patiënten met een voorgeschiedenis van gastro-intestinale bijwerkingen en met name ouderen moeten elk ongewoon gastro-intestinaal symptoom melden (vooral gastro-intestinale bloedingen), vooral in de eerste stadia van de behandeling.
Les patients ayant un antécédent d’effets indésirables gastro-intestinaux et notamment les sujets âgés doivent signaler tout symptôme gastro-intestinal inhabituel (surtout des saignements gastro-intestinaux), en particulier lors des premiers stades du traitement.
