De symptomen vertonen een karakteristiek gemengd- en wisselend beeld en omvatten een beginstadium van 'verdoving' met enige bewustzijnsvernauwing en verenging van aandacht, onvermogen prikkels te begrijpen en desoriëntatie. Deze toestand kan gevolgd worden door hetzij verdere terugtrekking uit de situatie (tot dissociatieve stupor toe - F44.2) of door agitatie en hyperactiviteit (vluchtreactie of fugue). Autonome tekenen van panische angst (tachycardie, zweten, blozen) zijn gewoonlijk aanwezig. De symptomen verschijnen doorgaans binnen minuten na de inwerking van de stressveroorzakende prikkel of gebeurtenis en verdwijnen in twee tot drie dagen (dikwijls in uren). Er kan gedeeltelijke of volledige amnesie (F44.0) voor de episode zijn. Cet état peut être suivi d'un retrait croissant vis-à-vis de l'environnement (pouvant aller jusqu'à une stupeur dissociative - voir F44.2), ou d'une agitation avec hyper-activité (réaction de fuite ou fugue). Le trouble s'accompagne fréquemment des symptômes neuro-végétatifs d'une anxiété panique (tachycardie, transpiration, bouffées de chaleur). Les symptômes se manifestent habituellement dans les minutes suivant la survenue du stimulus ou de l'événement stressant et disparaissent en l'espace de deux à trois jours (souvent en quelques heures). Il peut y avoir une amnésie partielle ou complète (F44.0) de l'épisode.
