Cette catégorie concerne des enfants ayant un retard mental prononcé (Q.I. inférieur à 34) associé à une hyperactivité importante, une perturbation majeure de l'attention et des comportements stéréotypés. Les médicaments stimulants sont habituellement inefficaces (alors qu'ils peuvent être efficaces chez les enfants ayant un Q.I. normal) et peuvent provoquer une réaction dysphorique sévère (accompagnée parfois d'un ralentissement psychomoteur).
