Definition: This block contains a wide variety of disorders that differ in severity and clinical form but that are all attributable to the use of one or more psychoactive substances, which may or may not have been medically prescribed. The third character of the code identifies the substance involved, and the fourth character specifies the clinical state. The codes should be used, as required, for each substance specified, but it should be noted that not all fourth character codes are applicable to all substances. Identification of the psychoactive substance should be based on as many sources of information as possible. These include self-report data, analysis of blood and other body fluids, characteristic physical and psychological symptoms, clinical signs and behaviour, and other evidence such as a drug being in the patient's possession or reports from informed third parties. Many drug users take more than one type of psychoactive substance. The main diagnosis should be classified, whenever possible, according to the substance or class of substances that has caused or contributed most to the presenting clinical syndrome. Stresses that more efforts are needed to control the use of antimicrobials in human and veterinary medicine; strongly disapproves of the regular prophylactic use of antimicrobials in animal husbandry; endorses the Council conclusions of 22 June 2012 which call on the Member States to limit the prophylactic use of antimicrobials to cases with defined clinical needs and to limit the prescription and use of antimicrobials for herd treatment of animals to cases where a veterinarian has assessed that there is a clear clinical and, where appropriate, epidemiological justification to treat all animals; stresses that livestock farming and aquaculture should focus on disease prevention through good hygiene, housing and animal husbandry, as well as through strict bio-security measures, rather than through the prophylactic use of antimicrobials; believes that controls on food imports from non-Member States should be enhanced, in particular in light of the risk that such imports contain irregular traces of antimicrobials;

Underlines that the key objective of any AMR strategy is to maintain the efficiency of existing antimicrobials by using them responsibly at the correct therapeutic level only when strictly necessary and prescribed over a specific time at the appropriate dosage, and reducing the use of antimicrobials in general and especially of Critically-Important Antimicrobials (CIAs) in human and veterinary medicine, thereby also taking into account the OIE List ; underlines the absolute need for an active holistic approach, based on a 'One Health' perspective, in order to achieve better and more efficient coordination between the human health sector and the veterinary sector; calls for enhanced surveillance of the use of antimicrobials in babies and young children, as well as in clinical treatment, where there is a need to control and measure antimicrobial use;

The inappropriate use of therapeutic antimicrobials in human and veterinary medicine, the use of antimicrobials for non-therapeutic purposes as well as the pollution of the environment by antimicrobials is accelerating the emergence and spread of resistant microorganisms. The consequences are severe:

Action n° 2: Strengthen the regulatory framework on veterinary medicines and on medicated feed via the review package foreseen for 2013, in particular: To ensure appropriate warnings and guidance on the labels of veterinary antimicrobials. To consider restrictions on the regular or the off-label use of certain new or critically important antimicrobials for humans in the veterinary sector. To consider amending the rules for the advertisement of veterinary antimicrobials. To revisit the authorisation requirements in order to sufficiently address the risks and benefits of antimicrobial medicines.

Selling of antimicrobial agents without a prescription is considered a relevant source of inappropriate antimicrobial use in at least seven countries, although no country was able to estimate the current proportion of all antimicrobial agent sold without prescriptions.
