Definition: This block contains a wide variety of disorders that differ in severity and clinical form but that are all attributable to the use of one or more psychoactive substances, which may or may not have been medically prescribed. The third character of the code identifies the substance involved, and the fourth character specifies the clinical state. Among the varied phenomena of the syndrome, patients complain most frequently of loss of emotions and feelings of estrangement or detachment from their thinking, their body, or the real world. In spite of the dramatic nature of the experience, the patient is aware of the unreality of the change. The sensorium is normal and the capacity for emotional expression intact. Depersonalization-derealization symptoms may occur as part of a diagnosable schizophrenic, depressive, phobic, or obsessive-compulsive disorder. In such cases the diagnosis should be that of the main disorder.

Although less severe psychosocial stress ( life events ) may precipitate the onset or contribute to the presentation of a very wide range of disorders classified elsewhere in this chapter, its etiological importance is not always clear and in each case will be found to depend on individual, often idiosyncratic, vulnerability, i.e. the life events are neither necessary nor sufficient to explain the occurrence and form of the disorder. In contrast, the disorders brought together here are thought to arise always as a direct consequence of acute severe stress or continued trauma. The stressful events or the continuing unpleasant circumstances are the primary and overriding causal factor and the disorder would not have occurred without their impact. The disorders in this section can thus be regarded as maladaptive responses to severe or continued stress, in that they interfere with successful coping mechanisms and therefore lead to problems of social functioning.

Taking into account the specific nature of direct and indirect taxes, the Commission has prepared several modifications that should, in the final stage, lead to three autonomous legal instruments. Firstly, the Council Regulation 1798/2003/EC on administrative co-operation in the field of value added tax and repealing Regulation No. 218/92. Secondly, the recently amended Council Directive 77/799/EEC concerning mutual assistance by the competent authorities of the Member States in the field of direct taxation and taxation of insurance premiums.
Compte tenu de la nature spécifique des impôts directs et indirects, la Commission a préparé plusieurs modifications qui, à terme, devraient conduire à trois instruments juridiques autonomes: primo, le règlement (CE) n° 1798/2003 du Conseilconcernant la coopération administrative dans le domaine de la taxe sur la valeur ajoutée et abrogeant le règlement n° 218/92; secundo, telle que récemment modifiée, la directive 77/799/CEE du Conseil concernant l'assistance mutuelle des autorités compétentes des États membres dans le domaine des impôts directs et des taxes sur les primes d'assurance; enfin, la proposition à l'examen, qui vise à établir des règles relatives à la coopération dans le domaine des droits d'accises.

In the context of the two Commission proposals concerning 77/799/EEC, Council consulted Parliament on the change of legal basis. Parliament has shown that this type of administrative cooperation does not affect material tax law, but rather seeks to ensure that the single market works well. There is therefore no justification for the Council to change the legal basis.

By letter of 28 July 2003 the Commission submitted to Parliament, pursuant to Articles 251(2) and 95 of the EC Treaty, the proposal for a European Parliament and Council directive on amending Directive 77/799/EEC concerning mutual assistance by the competent authorities of the Member States in the field of direct and indirect taxation (COM(2003) 446 – 2003/0170(COD)).
