Ce diagnostic ne doit être porté que dans les cas où on a la preuve d'un changement manifeste et durable des modes de perception, de relation ou de pensée concernant l'environnement ou soi-même. La modification de la personnalité doit être significative et être associée à un comportement rigide et mal adapté, absent avant la survenue de l'événement pathogène.
